Your respiratory system is the network of organs and tissues that help you breath. It’s a complex system that helps your body absorb oxygen from the air so your organs can work. It also cleans waste gases, like carbon dioxide, from your blood.
Common problems include allergies, diseases or infections, lung conditions, fluid in the lungs (pneumonia), narrowing of the bronchial tubes (emphysema), inflammation of the mucus membranes (colds) and breathing problems due to muscle weakness or obesity.
The health history for the respiratory system should be obtained after obtaining a detailed medical history. A thorough review of this information will provide an accurate assessment of your current status.
In addition, it will assist your doctor in determining whether there are any areas where you may need further testing, treatment or consultation with a specialist. It will also allow him or her to determine what type of care might best suit your needs.
You can obtain a general idea about your overall health by answering questions relating to lifestyle factors, family history, occupation, hobbies, etc. You will also want to discuss your diet, exercise habits, nutritional intake, medications, alcohol use, smoking, exposure to chemicals and toxins, travel, stress levels and environmental exposures.
Once you have completed all these items, ask your healthcare provider if he or she thinks you would benefit from additional testing or specialized evaluation by a physician. These tests could include x-rays, electrocardiograms, lab studies, echocardiography and pulmonary function tests. They may also be used to evaluate organ systems, such as the digestive tract, eyes, ears, skin, nervous system, endocrine glands and musculoskeletal system.
It is important to remember that the health history form does not replace a complete physical examination nor should it take the place of discussing symptoms and concerns with your primary care physician.

1.  What do you know about your personal and family health?
a.  List your parents’ birthplaces and their date of death. If they died before age 65, list their ages at time of death. Also, indicate if they smoked or had high cholesterol.
b.  Did anyone close to you have asthma, emphysema, pneumonia, or any chronic respiratory disease? If yes, describe how long ago did they die? Was it sudden or gradual?
c.  Have you ever been diagnosed with asthma, emphysema, or COPD? If no, list why not?
d.  Describe your own tobacco use – cigarettes only, cigars only, both? When was it started? How often do you smoke now? Has it changed over time? Why?
e.  Do you drink alcoholic beverages? If yes, list number of drinks per week/month/year. Alcohol consumption has been linked to many types of cancer, liver damage, pancreatitis, gastritis, ulcers, hepatitis, abnormal cell growth, brain tumors, strokes, dementia, heart attack, hypertension, diabetes, osteoporosis, and premature menopause.
f.  Do you currently use nonprescription drugs or herbal remedies? Which ones?
g.  List who live in your household – spouse, children, relatives, friends.
h.  Are you taking prescription medication regularly? Name them and when you last took a new one. Indicate dose and frequency.
2.  What do you know about your occupational health?
a.  List your occupations.
b.  Were you exposed to dusts, fumes, cigarette smoke, or other hazards? Tell us about each exposure.
c.  Do you have secondhand smoke or passive smoking in your home?
3.  What do you know about your mental health?
a.  List major stresses in your life.
4.  What do you know about your social environment?
a.  List your living arrangements.
5.  List your recreational activities.
6.  What do you know about your nutrition?
a.  List foods eaten daily.
7.  What do you know about your personal hygiene?
a.  List hair style.
8.  What do you know about your dental health?
a.  List teeth lost by accident or surgery.
9.  What do you know about your weight control?
a.  List any diets you’ve followed.
10.  What do you know about your sleep patterns?
a.  List hours slept nightly.
11.  What do you know about your sex life?
12.  List sexual practices.
13.  Does your partner have similar health issues or behaviors?
14.  What do you know about your emotional state?
15.  Do you feel depressed or anxious often?
16.  List sources of energy.
17.  List positive things happening in your life.
18.  List negative things happening in your life.
19.  Is there anything else you think we should know about?
20.  Any comments?
If you answered YES to any of the above questions then you may want to consider seeking professional advice regarding those particular areas.
For example, if you were using marijuana heavily or drinking more than you said previously, you may have some issues related to liver failure, pancreatitis, gastritis, ulceration, hepatitis, abnormal cell growth, brain tumor, stroke, dementia, heart attack, hypertension, diabetes, osteoporosis, and premature menopause.
